Ayurvedic Management of Psoriasis in a 32-Year-Old Male

Patient Profile & Presentation

  • Patient: 32-year-old male

  • Primary Complaint: Chronic scaling, severe itching (Pruritus), thick erythematous plaques over the extensor surfaces of the elbows, knees, and lower back, persisting for over three years.

  • Ayurvedic Diagnosis: In Ayurveda, chronic plaque psoriasis is primarily correlated with Eka Kustha or Kitibha Kustha, driven by a deep-seated tridoshic imbalance—predominantly involving Vata and Kapha, with severe vitiation of Rakta Dhatu (blood tissue) and accumulation of deep-rooted metabolic toxins (Ama).

Clinical Approach & Treatment Protocol

Managing recalcitrant skin disorders requires a comprehensive, multi-layered protocol that cleanses the body internally, pacifies local inflammation, and restores skin barrier integrity. The treatment plan was structured into phases:

  1. Internal Medication (Classical & Self-Prepared Formulations):

    • Kaishora Guggulu: A classical formulation utilized to purify the blood (Rakta Shodhana), reduce inflammation, and promote the healing of psoriatic lesions.

    • Custom Self-Prepared Herbal Decoction (Kwath): Prescribed alongside classical medicines, this custom blend featured blood-purifying and bitter herbs like Manjistha (Rubia cordifolia), Khadira (Acacia catechu), and Neem (Azadirachta indica) tailored to the patient’s specific Prakriti.

  2. Therapeutic Enema (Matra Basti):

    • Recognizing that chronic skin disorders involve aggravated Vata affecting the deeper tissues, Matra Basti (therapeutic oil enema using medicated oils such as Mahatiktaka Ghrita or Sahacharadi Taila) was administered periodically.

    • Why it helped: Matra Basti acts directly on the root site of Vata dosha (the colon), profoundly calming the nervous system, pulling deep-seated toxins out through the eliminatory channels, and nourishing the systemic tissues from within.

  3. External Application:

    • Custom Medicated Taila: The patient was advised to locally apply a self-prepared, soothing herbal oil infused with Neem, Haridra (turmeric), and Aragvadha to alleviate dryness, reduce scaling, and control intense itching.

Outcome & Follow-Up

Over a period of 4 months of consistent internal therapy, specialized Matra Basti administration, and strict adherence to a Pitta-Vata pacifying diet (avoiding fermented, sour, and excessively salty foods), the patient experienced remarkable recovery. Scaling reduced by over 80%, severe itching subsided completely, and skin texture over the elbows and knees showed near-complete clearance without any adverse systemic side effects.
