Condition: Pilonidal Sinus (Nadi Vikara)

History: The patient presented with chronic pain, recurrent swelling, and persistent seropurulent discharge over the natal cleft (tailbone area) for over a year. He had previously undergone conservative courses of antibiotics with temporary relief, but the sinus tract repeatedly flared up.
Clinical Presentation & Evaluation

Chief Complaints: Throbbing pain during sitting, localized swelling, and intermittent discharge of fluid and blood from a primary sinus opening with a secondary branching tract.

Clinical Findings: Local examination revealed an active pilonidal sinus tract with surrounding inflammation. Seeking an alternative to extensive surgical excision and long recovery windows, the patient opted for Ayurvedic Kshar Sutra therapy.
Treatment Protocol & Management
The patient was treated under a comprehensive Ayurvedic clinical regimen:

Kshar Sutra Application: A medicated alkaline thread (Kshar Sutra) was successfully maneuvered and tied through the primary and secondary sinus tracts. Weekly outpatient visits were conducted for routine thread changes (Pratisaran), which simultaneously cut, cleaned, and healed the tract.

Duration of Therapy: The active Kshar Sutra cutting and debridement phase required 8 weeks to completely lay open and clear the sinus tract.

Wound Healing Phase: Following the removal of the thread, an additional 3 weeks were dedicated to complete epithelialization and absolute closure of the residual wound.

Supportive Regimen Advised:

Oral Ayurveda Medicines: Internal herbal formulations were prescribed to purify the blood (Raktashodhaka), improve digestion (Agni), and accelerate tissue healing.

Sitz Bath: Warm water sitz baths infused with antiseptic herbs were advised daily to maintain local hygiene and ease inflammation.

Local Dressing: Regular antiseptic and medicated oil dressings were maintained post-thread change.

Dietary Precautions: A high-fiber diet, proper hydration, and the avoidance of heavy, spicy, or junk foods to prevent digestive sluggishness and support recovery.

Results & Conclusion
The patient experienced consistent, complication-free recovery without requiring hospitalization or bed rest. The sinus tract was completely eradicated, and the wound achieved total structural closure by the end of the 11th week (8 weeks of active therapy + 3 weeks of secondary healing). During subsequent follow-ups, the area remained completely healthy with zero signs of recurrence.
